Guyana Cultural Association’s Summer Workshop Series Dress Code Disclaimer and Guidelines
We are excited to welcome your child/children to our summer workshop at St. Stephens Lutheran Church and to help maintain a respectful, safe, and comfortable environment for all participants, we kindly ask that you review and adhere to the dress code outlined below.
Disclaimer:
This workshop includes indoor and light outdoor activities such as crafts, games, music, dancing and learning sessions. All students should come dressed in comfortable, modest, and activity-friendly clothing. As we are in a church facility, we ask that their attire be respectful of the space.
Dress Code Rules:
Modest and Respectful Attire
Clothing must cover the midriff, chest, and undergarments at all times.
No crop tops, low-cut shirts, spaghetti straps, or see-through garments.
Shorts and skirts should reach at least mid-thigh when standing.
Sleeveless shirts should have straps at least two fingers wide.
No clothing with inappropriate images, language, or references (e.g., violence, profanity, pop culture with suggestive themes).
By enrolling your child/children in the workshop, you acknowledge that you’ve read and agree to follow these dress code guidelines. If a child arrives in attire that does not meet the standards, a parent may be contacted to bring a change of clothing.
We appreciate your cooperation in helping us provide a safe and respectful space for all children to grow, play, and learn together.
CONSENT FOR USE OF PHOTOGRAPHS, VIDEOS- GCA Caribbean Heritage Workshop Series (SWS)
I hereby irrevocably authorize and grant GCA-SWS/GACC and its employees/volunteers the right to photograph or video my dependent and use the photo and/or other digital reproduction of his/her physical likeness
for publication processes, whether electronic, print, digital or electronic publishing via the media or internet for the workshop.
I hereby release Guyana Cultural Association of New York Inc (GCA), Caribbean Heritage Summer Workshop Series (SWS) and Guyana Arts & Cultural Center (GACC) from any infringement or violation of personal and or property rights, of any sort whatsoever, based upon the use of the recordings and images. I
acknowledge that GCA-SWS/GACC own and shall own all rights, titles and interest (including copyright) to the recordings and photographs.
I warrant that I have full power to enter into this release and the terms of this release do not in any way conflict with any existing commitment on my part. Reference to the "recordings and photographs" in this release includes any and all edited video versions made by GCA-SWS/GACC and further, include any previously recorded material of me/my child as well as photographs taken by GCA-SWS/GACC
GENERAL INFORMED CONSENT & ACKNOWLEDGEMENT
I hereby certify that the above named child is physically able to participate in all classes at
GCA-SWS/GACC and that I know of no physical impairment which would in any manner limit his/her participation in such a program.
I hereby give my approval for my child's participation in any and all activities prepared by
GCA-SWS/GACC during the GCA Caribbean Heritage Summer Workshop Series and other activities of SWS. In exchange for the acceptance of said child's candidacy by GCA-SWS/GCA, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless
GCA-SWS/GACC and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising from activities of GCA-SWS/GACC.
I recognize that this release means that I am giving up, among other things, rights to sue
GCA-SWS/GACC, its respective entities, administrators, faculty members, employees, agents or students for injuries, damages or losses that my child may incur.
